Pay Balance/Co-Pay

Our new billing system requires a card on file for all patients

Both parts below required for all patients:

1) Click "Pay Balance/Co-Pay" Button and click "Save my information for a faster checkout" -  to keep card on file.

2) Click the "CC Auth Form" Button for the Credit Card Authorization form and sign with your mouse or finger.
